It is the kissing bug, a small blood-sucking bug that hides in cracks in walls, thatched roofs and dark corners of precarious housing. Its medical importance lies in its ability to transmit <em>Trypanosoma cruzi<\/em>, the parasite responsible for Chagas disease.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What is the kissing bug and where does it live?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The kissing bug belongs to the family Reduviidae, subfamily Triatominae. The most studied species and of greatest epidemiological relevance is <em>Triatoma infestans<\/em>, although there are more than 140 species of triatomines distributed mainly in the American continent. It receives different names depending on the country: kissing bug in Argentina, Bolivia, Chile, Peru and Uruguay; kissing bug in Mexico; chipo in Venezuela; and barbeiro in Brazil.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This insect measures between 2 and 4 centimeters in its adult stage, has a flattened dark brown body with lighter bands on the edges of the abdomen, and has functional wings. It inhabits temperate and warm zones, with a preference for houses built with adobe, wood, cane or straw, materials that offer multiple cracks where they can take refuge during the day.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Nocturnal behavior and way of feeding<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The kissing bug is strictly hematophagous: it feeds exclusively on the blood of mammals, birds and reptiles. Its activity is nocturnal. During the night it leaves its shelters and approaches sleeping people or animals, attracted by the body heat and carbon dioxide they exhale.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Its bite is practically painless, since its saliva contains anesthetic and anticoagulant substances. This allows it to feed for several minutes undetected, ingesting an amount of blood that can exceed several times its own weight. After feeding, it usually defecates near the site of the bite, and it is precisely in its feces that the parasite that causes the disease is found.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Chagas disease: a silent disease<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Chagas disease was described in 1909 by the Brazilian physician Carlos Chagas. It is caused by the protozoan <em>Trypanosoma cruzi<\/em>, which enters the human body when contaminated feces of the kissing bug come into contact with the bite wound, mucous membranes or small lesions on the skin, usually by scratching unconsciously.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The disease has two distinct phases:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Acute phase:<\/strong> occurs in the first weeks after infection. It can be asymptomatic or present fever, malaise, swollen glands and, in some cases, swelling at the site of the bite, known as Romagna&#8217;s sign when it affects the eyelid.<\/li>\n\n\n\n<li><strong>Chronic phase:<\/strong> after the acute phase, the parasite remains in the body for years or decades without generating obvious symptoms. Serious complications, mainly cardiac, such as Chagas cardiomyopathy, arrhythmias and heart failure, develop in approximately 30% of those infected. It can also affect the digestive system, causing enlargement of the esophagus or colon.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Magnitude of the health problem<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">According to data from the World Health Organization, it is estimated that between 6 and 7 million people are infected by <em>Trypanosoma cruzi<\/em> in the world, mostly in Latin America. Chagas disease causes around 10,000 deaths per year and is considered a neglected disease, since it mainly affects rural populations with fewer resources.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other routes of transmission, in addition to vector, include blood transfusion, organ transplantation, congenital transmission from mother to child during pregnancy and, to a lesser extent, the ingestion of food contaminated with the parasite.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Prevention and control<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The control of the kissing bug depends to a large extent on the improvement of housing conditions and sustained programs of fumigation with residual insecticides. Among the most effective measures are:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Plaster walls and ceilings to remove cracks and shelters.<\/li>\n\n\n\n<li>Keep the bedrooms and storage rooms clean and tidy.<\/li>\n\n\n\n<li>Separate the corrals and chicken coops from the houses.<\/li>\n\n\n\n<li>Use mosquito nets on beds in endemic areas.<\/li>\n\n\n\n<li>Periodically inspect homes for insects or their feces.<\/li>\n\n\n\n<li>Guarantee serological control of blood intended for transfusions.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Early diagnosis is critical. Treatment with antiparasitic drugs such as benznidazole and nifurtimox is most effective when applied in the acute phase or in children and young people, since in advanced stages its effectiveness decreases.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">A threat that persists<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Although several countries have been successful in interrupting vector transmission of <em>Triatoma infestans<\/em> through sustained regional programmes, kissing bug remains a health challenge in many rural areas of the continent.
